ANNUAL DUES STATEMENT 2010
Life Non-Active, Retired or Honorary Members do not pay annual dues. This form should be used for any change of address. Thus, please disregard this notice except for changes of address if you are in one of those categories. Change of Membership Category Requests for changes to membership category can not be made online but can be submitted via e-mail, fax or by US Mail only using the due statement form. Student Members who have graduated from a training program should transfer to the appropriate category of membership at this time. Note: Members residing outside the geographic boundaries of the Southwest Society of Periodontists may apply for NON-RESIDENT membership. The boundaries of the society include the states of Arkansas, Colorado, Louisiana, Nebraska, Oklahoma and Texas. Notice All categories of dues are due on January 1st each year and are considered delinquent if unpaid on April 1st each year. After April 1st, a $50 penalty will be assessed to those members whose dues are still unpaid. Those members whose dues are unpaid on June 1st each year will be removed from membership in the Southwest Society of Periodontists. The date of payment will be determined by the date of postmark. |
